*UPDATE* I heard back from my contact at 3M this morning (Feb 7th) and this is what I was told:
My apologies – I just got a message saying that including Canada in the sampling program was an error. The program is only for US residents.

*NOTE* We are aware at this time, the form is only accepting 5 digits for postal code instead of the required 6. I have reached out to my contact at 3M and let them know about the problem. This is the response I received:
I’ve tracked down the issue – this sample program is being run by 3M in the US. I’m working on contacting our counterparts in the US to have this problem fixed – I can’t put a timeline on it, but the US often takes a few days to fix problems like this. Let’s hope for the best!
